My appointment was with Lydia, as always she was very polite, took the time to explain exactly what she would be doing during the appointment and gave me further advice on dental care going forward.

Appointment

Filling out patient details (that have not changed) takes far too long. There should be a single question asking if any details have changed. If the answer is No it should go straight to the bottom of the form for signature and submission. If the answer is Yes you should be able to just amend those that have changed.
