Thank you or your feedback, we appreciate it sometimes feel like a lot of forms to complete, we have tried to improve the system with online access to allow you to complete this at home or you can fill them in on arrival at your appointment.
It is essential that we check at every appointment that your contact details are still the same so we can easily keep in touch regarding your appointments and for any other unforeseen need to contact you.
We are required to keep accurate and updated medical notes so that we can treat you correctly and safely, often if we ask ' has anything changed' some people forget a small medication change or something else that has changed but thought we knew so we have to be thorough for your safety.
The NHS form must be signed at the start of any new treatment, this form is sent off to the NHS with every treatment completed allowing the NHS to see what work has been carried out and to pay the Dentist accordingly,
I can reassure you all of this information is secure and not shared or sold with anyone, ( the NHS form is securely sent to the NHS board only.)
